Medical Director:
Approves the plan of care and works with the hospice team. The hospice Medical Director is available to the attending physician, patient, and hospice care team as a consultant and resource.
Hospice Skilled Nurse:
Assess the patient’s overall condition through dialogue, and examine and review past treatments and medications to manage symptoms to ensure comfort.
Medical Social Worker:
Improves Social, emotional, and functional status. Works with patients to facilitate services and enhance the coping skills of family and other caregivers.
Chaplain:
Provides spiritual guidance and support to both the patient and family.
